Background: You are a community nutritionist at the Department of Social Services serving in the neighborhood of (insert a place you have volunteered or your home neighborhood) ___________________. Your organization is looking for funding for a proposal to direct nutrition-related programming for the fiscal year based on the findings of your local Community Needs Health Assessment Report. For the purposes of this assignment, you can focus on one or two of the nutrition related findings to address in your programming. Your report is a formal report, avoid personal opinions and pronouns.

Step 1 – Define your community and define your population. Include the neighborhood description and SPA planning area. Identify six or more nutrition related findings within the Community Needs Health Assessment for your community and the population you are serving. Data should include at least two nutrition-related intake data statistics (soda consumption, fast food consumption, food insecurity/skipped meals, fruits and veg consumption etc), at least two health data statistics (leading causes of death, obesity rates, rates of diabetes, hypertension etc), and include at least two social ‘determinants of health’ data statistics (income levels, education, poverty levels, primary languages race and ethnicity etc). Compare your data with city/county/state or national averages., and/or compare and contrast your data within ethnic groups etc. Statement of the Problem: define the main nutrition-related problem in your community, (food insecurity, obesity, diabetes etc) Approx 2-3 paragraphs – (use your own words, cite using APA formatting).
Step 2 – Plan your intervention. Set goals and objectives for the intervention. Focus on the target audience. Consider changes in health outcomes.

Step 3 Brainstorm the types of changes you want to foster. Ask yourself, what knowledge, skills, and attitudes need to be developed to change actual behavior?
Step 4 Develop and describe TWO levels of intervention for your community. (Examples in Table 5.4)

Step 6 Describe an Outcome Evaluation for your program to determine how the program will measure changes (behavioral, anthropometric, clinical, knowledge, lab etc)
Step 7: Create a simple pre and post survey tool based to measure changes. Include demographic data (age, education levels, income etc) plus at least 7 questions that measure changes in health and/or behavior.
