Grading Criteria for Physical Assessment Paper and SOAP Note
Included are 3 sample student papers. Not all students received a 100% grade, but all received an A on their papers.
Remember that for a SOAP note written by a nurse, you must use a nursing diagnosis. You are not allowed to make a medical diagnosis as it is not within the RN scope of practice – only the nurse practitioner scope.
The physical exam write-up is to be done on your (same) partner who is pretending to be a client with a disease or disorder of your choice.
Please use proper, accepted medical terminology when describing your physical findings. You may abbreviate and do not have to use full sentences. This is not a group paper- you must write an individual paper.
