Pt. diaphoretic, anxious and disoriented to time and place. Pt. appears to be speaking to someone across the room that isn’t there.
Pertinent Health HistoryHealth history given by pt.’s mother: Hx of alcoholism. Last drink about 24 hours ago. Pt. has been trying to stop. Started getting “shaky” so he went outside to get some air.
No hx of seizures or hypertension.
NKDA
Not currently on medications.
