Patient is 4 years old girl who is a very healthy child. She lives with both parents and a bigger sister. She is currently attending VPK classes in which she gets to enjoy in a daily basics. She interacts with her family and friends with enthusiasm. Mom stated She still using a baby bottle twice a day and on the transition of using cups instead. She enjoys playing with her older sister and get along very well. She does her homework with a little help and she participates in class smart in class her mom says. Caroline is a very active child who uses a lot of expression when she talks. Caroline started to walk at 10 months and to eat by herself at 23 months. She potty trained at her 26 months and she said her first words at 9 months. Mom said she is a very alert child and follow instructions well but sometimes get frustrated when she finds something difficult she can’t do. She is a helper and try to help mommy when she needs to clean the house. When she goes to the park with her sister she likes to socialize with other kids around her very easy and interact with adults sometimes. Her mom said she loves to paint at the house. She is very creative and uses a lot of her imagination when painting. Because she is very competent child and like to do things the best way possible like a perfectionist the target improvement is to help her to do the same thing over and over until she feels confident she can do it better next time
