Burn patient papulation and patient with needs of wound care in the unit I work, are more susceptible to CAUTI than other patient because of their needs and how their immune systems are compromised due to burn or unhealed wound. CAUTI is an enormous risk to UTI or Urinary Track Infection. Decreasing any hospital acquired infections plays a great role in reducing overall unpleasant impact of treatments and help staff to meet patient’s expectation and satisfaction.

The AHRQ has numerous resources to enhance catheter results. A standard catheter care once a shift and PRN, has proven to be effective in decreasing the risk of CAUDI and therefore UTI. Education in the first place when the catheter is certainly necessary, can also help to lowering overall exposure, and the final call to use a catheter would be depending on especial requirements, like wounds at perineal skin or urine retention or critical fluid management in patient with cardiac issues.

There are many good tools that evidenced based studies have expanded and endorse them. At the Medical University of South Carolina, CAUTI was lowered from 8.6 percent to 1.1 percent by implantation of an AHRQ patient safety. The study on catheter associated infection done by Dr, Sanjay Saint (2013), he confirms that demonstrates that utilizing preset justifications for catheter use reduced the risk of CAUTI. He also clarifies that one of the obstacles to this is the consistency of the instruction being provided, which is why it would be advantageous to use the AHRQ advice of a poster.

Three modules—implementation, sustainability, and resources—make up the toolkit that AHRQ, provides for (CAUTI) catheter-associated urinary tract infections. These modules can be used by a hospital unit to train team members in the use of CUSP, or Comprehensive Unit-based Safety Program, to prevent CAUTI. By utilizing this toolkit, healthcare providers can develop or reinforce a culture of patient safety that will considerably lower incidence of CAUTI and other dangers related to the use of indwelling catheters. (“AHRQ Safety Program: Reducing CAUTI in Hospital Patients”) There are links to resources that can be used to instruct your unit as a leader and put this safe practice into action at your facility are provided in each of the modules mentioned above. Reducing CAUTI in Hospital Patients: AHRQ Safety Program. (n.d.).

All of the following characteristics of competencies in QSEN are applicable to my education level.

Patient-centered care involves giving attention to and taking into consideration the patient’s values, concerns, and goals. Specifically, being able to function as a team member while taking into account the team’s benefits and disadvantages and Act with honesty, constancy, and respect for other people’s opinions.

Evidence-based practice, also known as practice that considers the unique needs of each patient and basing decisions on clinical knowledge and evidence and Recognize the necessity of ongoing, knowledge-based improvement in clinical practice. Where I work, we use a lot of EBP for our patient and their dressing. While their dressing are changed every day, based on EBP and team decision, as a burn nurse I change my patient’s dressing to what is more beneficial to them.

Team work, an everyday component of the practice, quality improvement attempts to improve all aspects of the work. I think team work apply to all the level of education. Working in health care is all about helping each other out and team work.

Quality improvement, make care processes clear using tools (such flow charts and cause-and-effect diagrams). As a burn nurse, I use burn diagram for my patients all the time.

Safety, with the use of proper techniques to minimize risk whenever possible. As a nurse I evaluate my patient safety using different tools every shift. I also do my safety check at the beginning of each shift in case of code.

Technology, the application of technology to enhance clinical judgment, lower risk, and advance learning. We all use technology and record our judgments and tasks we do every shift using informatics. That helps with preventing many errors. For example, with Alaris pumps, we prevent many medication errors.

So as an RN all the above QSEN competencies apply to my daily work life.

References:

Quality and Safety Education for Nurses Institute.
