There are a variety of community resources that nurses should integrate as part of care coordination alongside ethical considerations that should inform their practice. On the same note, policy issues, and safety concerns, alongside cultural norms, should also be adopted in the efforts and the process towards the provision of smooth and seamless care. Thus, the role of nurses cannot be downplayed since they are responsible for providing required knowledge while also ensuring effective communication to help transfer this knowledge for seamless transitions of care. Drawing upon evidence-based practices is also vital for nurses as it ensures that they are better equipped to promote health alongside ensuring a conducive ecosystem is established that enhances the improvement and maintenance of the health of individuals and families. The resources highlight the need for a care coordination plan that contributes towards achieving and maintaining optimal health.<\/p>\n<p> Patient Case Scenario<\/p>\n<p> Mr. D is a 35-year-old electrician with severe depression. Following the pandemic outbreak, he lost his job at a local station as he maintained and repaired current surges within homes. The outbreak of the Covid-19 pandemic saw his company lose many clients, which created the need to downsize to manage the wages of its workforce and the limited orders received. This encounter was stressful as it meant that he would not manage to sustain his family, and this resulted in stress. His wife left him amidst the peak of the pandemic as she moved to her parent&#8217;s home with the hope of sustaining their three children. Mr. D has been isolated since the departure of his wife. He has become an alcoholic and is disinterested in activities he initially found interesting and intriguing. Mr. D cannot sleep, and he has lost his appetite. He has been developing suicidal thoughts recently. He has been diagnosed with severe depression and placed under suicide watch as he undergoes therapy.<\/p>\n<p> Patient-Centered Health Interventions<\/p>\n<p> A patient-centered health intervention would be vital for Mr. D. He has been diagnosed with depression which has significantly undermined his well-being. This care coordination plan helps enhance his well-being and address other underlying issues related to mental health illnesses. Healthy People 2030 lays out several quantifiable goals to enhance the state of people&#8217;s health and well-being across the United States. In addition, the initiative offers a variety of resources and tools to assist individuals and organizations in accomplishing these goals (OASH. 2022). These tools offer resources that are based on evidence (EBRs). The specific objective that is being pursued by this care coordination strategy is to increase the proportion of people who can receive treatment for their drug use disorders and mental health issues (OASH. 2022). This is a goal that fits in with Mr. D&#8217;s case.<\/p>\n<p> Best Practices<\/p>\n<p> The most effective technique to deal with mental health concerns is identifying the risk factors contributing to the best plan for conquering challenges. In this sense, genetic and biological traits are crucial risk factors since they highlight the need for therapies that healthcare stakeholders should adopt. As a result, risk factors such as the environment, drug, and substance abuse, as well as biological and sociocultural factors, all enhance the possibility of experiencing the aforementioned mental health difficulties (Hossain et al., 2020). Therefore, the stakeholder approach is to produce best practices that improve the containment of mental health therapies to promote potential outcomes. These activities are fundamental to best practices that promote holistic outcomes for people&#8217;s mental, emotional, and physical health and well-being (Girdhar et al., 2020). The holistic care approach focuses on the mind, body, and emotional factors to provide the best care plan and coordination environment. The primary intervention focuses on integrating caregiving methods with the needs of the target population or community to improve prevention tactics.<\/p>\n<p> A collaborative framework that brings together key stakeholders such as the community, target population, healthcare professionals, regulators, and patients can improve containment techniques. Coordination and collaboration support best practices, which outline how holistic care frameworks can handle specific circumstances. Establishing a mutually beneficial relationship between patients and their families is critical to developing the optimal plan. Understanding the risk factors in the context of biological, genetic, environmental, and cultural factors might help with this (Hossain et al., 2020). To guarantee that the population receives evidence-based interventions that limit predicted outcomes, the appropriate care coordination strategy must be designed with the assistance of all specialists and regulators. For example, the greatest healthcare setting is one where nurses and doctors can deliver clinical and therapeutic services to patients to receive comprehensive treatment (Biotteau et al., 2019). Many of the projected outcomes for patient care are expected to be based on the utilization of the most recent research, technological advancements, and best practices to improve the well-being of the targeted population by limiting exposure to and regulating risk factors.<\/p>\n<p> Objectives<\/p>\n<p> One mental health risk factor is illiteracy, as is a lack of awareness of the projected disorders and symptoms among the targeted demographic. Men are also more likely to conceal their mental health concerns and emotional suffering from their caregivers, undermining ideal health (Kamimura et al., 2018). The key concept for encouraging society to accept mental health awareness is to build a coordinated approach in which all stakeholders craft the best message. The primary goal of this care coordination strategy is to improve knowledge of mental health risk factors in various communities and the best resources and techniques for addressing them. According to social, economic, and other factors that cause individuals misery, as well as other perspectives, around one in every five Americans has varying degrees of mental health challenges (Girdhar et al., 2020). The applicable mental health preliminary care coordination plan models make it feasible to emphasize the best practices that protect the target population&#8217;s well-being.<\/p>\n<p> Public awareness campaigns will produce research-based materials, integrated and holistic care concepts, and efficient communications that will answer the population&#8217;s demand for information on the fundamentals of mental health and how to manage it in the long term. Identifying the mental health risk factors within a specific community using the resources and strategies at hand is part of this coordination plan (Girdhar et al., 2020). People rarely realize what causes their depressive episodes. Thus it is critical to have a strategic awareness of the mental health challenges they encounter. Contextualizing mental and emotional health sources can aid a strategic awareness campaign to improve understanding and appreciation of mental health risk factors. Identifying the risk factor includes targeting stakeholders who engage within community frameworks to influence responsiveness to behavioral methods. Mental health concerns are risk factors that may present at work, school, home, and in the community at large and may impact the identified patients&#8217; overall health and well-being (Biotteau et al., 2019). It aims to develop a network of stakeholders to improve the identification of risk factors and boost the identified patients&#8217; awareness levels to achieve the anticipated goals and objectives.<\/p>\n<p> Community Resources<\/p>\n<p> The use of community resources to address risk factors and mental health concerns is required for the fulfillment of the defined goals and objectives. A safe and efficient continuum of care involves processes and methods that use adjacent facilities, institutions, and resources to achieve the desired goals. In local communities, key services for drug abuse, religion, entertainment, financial and economic empowerment, and youth-related services are accessible (Saha et al., 2019). In addition, these organizations and services have networks that improve tactics and procedures for boosting local community mental health awareness to achieve the desired results.<\/p>\n<p> In addition to these organizations and service-related institutions, the project can use individuals with the knowledge, clout, and abilities to raise awareness of mental health, wellness, and well-being. Individuals such as educators, political officials, social and religious influencers, local celebrities, and local government representatives can be employed using these approaches. These groups bring security, clarity, trust, and seriousness to the care coordination strategy to ensure that the community is appropriately managed (Kamimura et al., 2018). Furthermore, schools, churches, playgrounds, practice fields, and community health centers may be critical. These websites are platforms for increasing awareness of and understanding major mental health issues. Better local community resource coordination is expected to boost the success of public health promotion.<\/p>\n<p> This care coordination plan is informed by a health continuum that strongly supports care coordination since it helps ensure a smooth and seamless treatment transition. Along with ethical considerations that guide their practice, nurses should incorporate several community services as part of care coordination. In keeping with this, efforts and processes to provide fluid and seamless care should also consider policy difficulties, safety concerns, and cultural values. As a result, nurses play an important role in ensuring that patients receive the necessary information and adequate communication to facilitate the knowledge transfer needed for smooth care transitions. This care coordination plan utilizes evidence-based practices, which are essential as it gives them the tools they need to better promote health and create an environment conducive to improving and maintaining people&#8217;s health.
