He wants to be seen one on one, “not in one of those groups.” He reports that his first admission to treatment was in an adolescent program when he was 13 years old. He was referred by the school counselor for smoking marijuana in the boys room in middle school. He states that he was in and out of trouble mostly for smoking marijuana throughout high school; never had many friends. He reacted badly to drinking alcohol, immediately getting in trouble, said he doesn’t like to drink, he barely drinks. No significant history with other substances. “Seen too many zombies to mess with anything else.” However, he has smoked marijuana continuously – daily, high doses. He completed high school and stated, “It wasn’t a very tough school – just show up.” Looking a little older than his stated age he secured a job as a doorman at an upscale apartment building in Manhattan and has been doing this at one place or another for the past 14 years. He wanted to go to college, get a better job “get on with life” but “it just never happened.” For the past 10 years he has been in and out of treatment, voluntarily, for Marijuana dependence. “I’m the original fiddler’s elbow” he states. “I meet about 9 criteria and no matter what I do I just keep going back to the weed.” He thinks it is maybe because work is so boring. He smokes marijuana and isolates endlessly, has no close friends, does not “hit it off real good with the ladies” because he thinks they see right away that he is just a “lazy old stoner.” He has a cat at his small apartment in the city. His parent’s both passed away “too young” he says and he has no family in the city. He presents as lonely, sad, and isolated. He states that he knows the weed is a problem but every-time he quits it’s like he doesn’t know what happens – he just goes back to using. “I wish I was straight; I’d join a church choir at Baptist and be normal, I could get right, find a good lady. How come I just keep smoking and never getting anything done?” He feels nothing is going to help and nothing is going to change. Lately he wonders why he is even living. Relying on your knowledge of the relapse literature read for this module, what do you think is going on with this client? Why does he keep relapsing? What is his level of Motivation?
