It is one of the most common infections in the outpatient primary care setting. Urinary tract infections are very common among women due to the anatomical structures of the female’s urinary system and its proximity to the vagina and the anus. UTIs are rare in males younger than 50 years of age but the prevalence increases in males older than 50 years (Hanna-Wakim et al., 2015). However, providers should be careful when treating UTIs in younger males as this group has a higher chance of developing resistance to antibiotic therapies (Hanna-Wakim et al., 2015).

Urine cultures are critical to the treatment and management of UTI in male patients; it will help to identify the adequate antibiotic therapy and help to prevent and manage future urinary tract infections.

Some causes of UTIs in males are prostatitis, epididymitis, orchitis, pyelonephritis, cystitis, urethritis, and urinary catheters. In 75 % of cases the male patient may present with complaints of painful urination or dysuria, frequency, or urgency and 33% will complain of dribbling, slow urinary stream, and acute onset of hesitancy (Tan & Chlebicki, 2016).

The outcome is usually benign, but UTIs can progress to renal scarring in early infancy, especially when associated with congenital anomalies of the urinary tract. Renal scarring may lead to complications in adulthood including hypertension, proteinuria, renal damage, and even chronic renal failure, which requires dialysis treatment.

A urology consult should be initiated along with a 10 to 14-day course of antibiotics in male patients who present with stable vitals and are not ill appearing. Antibiotic treatment considerations should include either of the class of antibiotics; fluoroquinolone, trimethoprim-sulfamethoxazole (TMP-SMZ), minocycline, or nitrofurantoin (Nicole et al., 2005). Treatment should be adjusted based on urinary culture results. Escherichia coli remains the most common uropathogen in acute community acquired uncomplicated UTIs and amoxicillin-clavulanate is useful as a first-line antibiotic (Nicole et al., 2005). Family physicians can manage most UTIs if guided by appropriate history, investigations, and appropriate antibiotics to achieve good outcomes and minimize antibiotic resistance.

REFERENCES

Tan, C.
