When you post this in the threads, it is acceptable to attach a Word Doc to the post after briefly listing the appendices you completed, due to the capabilities of this display in Canvas on the discussion board. Once you receive feedback from your faculty member, make all necessary changes and cut\/paste the content of this worksheet 6 of the appendices into the \u201cDNP Project Template\u201d, which is found in \u201cResources\u201d\u2014\u201cProject and Practicum Resources.\u201d For NR 702, it is required to complete the Appendices. Appendix A is required (optional for NR 702), as you will begin to complete the evidence table in navigation, continue in NR 702 and then complete in NR 705 with your literature synthesis. <\/p>\n<p> Directions for Use of the Individual Evidence Summary Tool<\/p>\n<p> This table should be started in navigation, continued in NR 702, and completed in NR 705. There will be additional research added in NR 709.<\/p>\n<p> Purpose<\/p>\n<p> This form is used to document the results of evidence appraisal in preparation for evidence synthesis. The form provides the EBP team with documentation of the sources of evidence used, the year the evidence was published or otherwise communicated, the information gathered from each evidence source that helps the team answer the EBP question, and the level and quality of each source of evidence.<\/p>\n<p> Article Number<\/p>\n<p> Assign a number to each reviewed source of evidence. This organizes the individual evidence summary and provides an easy way to reference articles.<\/p>\n<p> Author and Date<\/p>\n<p> Indicate the last name of the first author or the evidence source and the publication\/communication date. List both author\/evidence source and date.<\/p>\n<p> Evidence Type<\/p>\n<p> Indicate the type of evidence reviewed (for example: RCT, meta-analysis, mixed methods, quaLitative, systematic review, case project, narrative literature review).<\/p>\n<p> Sample, Sample Size, and Setting<\/p>\n<p> Provide a quick view of the population, number of participants, and project location.<\/p>\n<p> Findings That Help Answer the EBP Question<\/p>\n<p> Although the reviewer may find many points of interest, list only findings that directly apply to the EBP question.<\/p>\n<p> Observable Measures<\/p>\n<p> QuaNtitative measures or variables are used to answer a research question, test a hypothesis, describe characteristics, or determine the effect, impact, or influence. QuaLitative evidence uses cases, context, opinions, experiences, and thoughts to represent the phenomenon of project.<\/p>\n<p> Limitations<\/p>\n<p> Include information that may or may not be within the text of the article regarding drawbacks of the piece of evidence.
