The holistic plan of care will also include goals, holistic self-care interventions, challenges and opportunities for the older adult client, evaluation of the goals, and overall impact of the plan of care to improve outcomes.<\/p>\n<p>Below is an outline to guide the paper:<\/p>\n<table class=\" ic-Table--hover-row ic-Table ic-Table--striped\">\n<tbody>\n<tr>\n<td width=\"551\">Title page<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Introduction (includes the purpose of the paper\/what the reader should expect in the body of the paper)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Comprehensive Holistic Assessment (includes physical, psychological, environmental, and cultural\/ethical assessment data)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Analysis of Assessment Findings (includes notable age-related changes)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Goals (includes at least one short- and one long-term goal)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Holistic Self-Care Interventions (clearly identifies at least four)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Challenges and Opportunities (includes coaching strategies to assist the client)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Evaluation (includes measures to monitor goals &amp; impact of care plan to improve outcomes)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">Conclusion (summarizes the paper without adding new information -includes the overall impact of the plan of care)<\/td>\n<\/tr>\n<tr>\n<td width=\"551\">References (includes a minimum of five scholarly resources)<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>Submit an academic paper, with 8-10 pages of content (not including title &amp; reference pages), using proper APA style.
