It also provides a structured approach to document patient problems, interdisciplinary interventions, and outcomes at the point of care (Stanhope &amp; Lancaster, 2020). Below I chose one member of family A, Mister GS, to do an assessment, interventions, and outcomes following the Omaha system documentation.<\/p>\n<p> .<\/p>\n<p> PROBLEM: MENTAL HEALTH<\/p>\n<p> NURSING DIAGNOSIS-INEFFECTIVE COPING MECHANISM RELATED TO SITUATIONAL CRISIS AS EVIDENCE BY VERBALIZATION OF INABILTY TO COPE AND REPORTED LIFE STRESS.<\/p>\n<p> PROBLEEM RATING<\/p>\n<p> Concepts<\/p>\n<p> 1<\/p>\n<p> 2<\/p>\n<p> 3<\/p>\n<p> 4<\/p>\n<p> 5<\/p>\n<p> Knowledge<\/p>\n<p> \u2022<\/p>\n<p> Behavior<\/p>\n<p> \u2022<\/p>\n<p> Status<\/p>\n<p> \u2022<\/p>\n<p> According to the Omaha System Problem Rating Scale for Outcome Family A has a score of 5.<\/p>\n<p> Problem Rating Scale for Outcomes <\/p>\n<p> Knowledge: 2\u2014 minimal knowledge (did not know about the community resources for mental health crisis) <\/p>\n<p> Behavior:1\u2014Not appropriate behavior (cannot use problem-solving skills) <\/p>\n<p> Status: 2\u2014 Severe signs and symptoms (multiple episodes of high blood pressure)<\/p>\n<p> INTERVENTIONS AND OUTCOMES<\/p>\n<p> Subjective and objective data<\/p>\n<p> Nursing diagnosis<\/p>\n<p> Goals and objectives<\/p>\n<p> Nursing interventions<\/p>\n<p> Rationales<\/p>\n<p> Evaluations <\/p>\n<p> Subjective<\/p>\n<p> Mister GS reported having stress due to loss of jobs and frequent doctors\u2019 visits for episodes of high blood pressure due to stress.<\/p>\n<p> Ineffective coping mechanism related to situational crisis as evidence by verbalization of inability to cope and reported life stress (Gulanick, M., &amp; Myers, J. L. (2017).<\/p>\n<p> Within a month of therapy and counseling Mister GS will be able to explain and adopt coping strategies, will utilize available assistance.<\/p>\n<p> Assessment of Mister GS readiness to learn coping mechanism skills.<\/p>\n<p> Once Mister GS understands that there is a need to change th\u2022\u2022e approach to solve a problem, he will be open to receive advice about necessary changes and adjustments.<\/p>\n<p> The coping outcomes were met, Mister GS was able to identify several coping mechanisms and was utilizing them and was also going to therapy and counselling as per referral.<\/p>\n<p> Introduce one method at a time.<\/p>\n<p> A slow but steady plan ensures that Mister GS can easily practice these new coping skills without feeling overwhelmed.<\/p>\n<p> Refer Mister GS for counseling as necessary<\/p>\n<p> Arranging for referral will assist Mister GS in working with the system, and resource use helps to develop problem-solving and coping skills.<\/p>\n<p> REFERRAL<\/p>\n<p> The community psychiatrist nurse has contacted by phone the RPS Behavioral Health Response Team of Rockland County. It is a multidisciplinary team of mental health clinicians and technicians serving the community of Rockland. The team responds to requests for assistance or intervention with any number of behavioral health issues. These issues can range from serious emotional crisis to seeking guidance for behavioral health support. It is also a free service.<\/p>\n<p> It is important to discuss the benefit of seeking help through the community agency\/resource with Mister GS. Utilizing resources in the community it is a good place to start to reduce stress. I will discuss the strategies used by these agencies that may help them reduce or manage their stress such as: <\/p>\n<p> -Therapy and counselling, life coaching<\/p>\n<p> -Support groups<\/p>\n<p> -Meditation<\/p>\n<p> -Social support from close friends and family members<\/p>\n<p> CONCLUSION<\/p>\n<p> With the Omaha system I was able to assess one member of Family A, provide a teaching plan and an outcome. Mister G will need a lot of support from family members and friends, a change of lifestyle, and support groups in the community to overcome the stressful event of losing his job due to the pandemic.<\/p>\n<p> References<\/p>\n<p> Gulanick, M., &amp; Myers, J.
