In order to be considered for an academic dismissal appeal, you will need to write an appeal letter. I have included the content needed below:

 

Provide which courses you were unsuccessful and include reason(s) why you were unsuccessful.  Did you utilize any university or outside resources to assist you while in the program (tutoring, advising contact, etc.)?  What challenges/adversity lead to you being unsuccessful and how have they now been resolved?  Also, describe your plan for success including why you feel you should be reinstated.

Dear Academic Affairs department,

I am writing to request for a remediation for my Women and Newborn Health final exam which I failed due to Emotional Breakdown. I lost my Job which led to financial crisis and drained me emotionally because I could not meet financial needs. Regardless of the financial problem I was going through, I still studied and scored 68% in the first exam then did a remediation and met with my professor for exam review. I studied hard, did a study group and passed exam 2 with a score of 82. I studied and did enrichment quiz in preparation of ATI exam and passed with a level 3 (88.3%). The final exam I scored 70 and I was really disappointed I did not do well. For every course that I took before this semester, my grade has been good and I believe that I can do better with this course too. I understand it is my responsibility as a student to study and pass every exam but I would appreciate it if the committee reconsider me and allow me to retake the final exam. Thank you for taking the time to consider my request.
