M.M., who is a 54 year old single male presents to the ER reporting chest tightness. The pain started while he was running to catch-up with a co-worker. He describes previous episodes of chest pain resolving spontaneously with a brief rest. This episode has not resolved. He asks you “Am I having a heart attack? My father died of one about my age.” He also has a history of hypertension and possibly prediabetes. M.M. undergoes a cardiac catheterization procedure revealing 90% occlusion of the inferior coronary artery and a cardiac ejection fraction of 39%. He undergoes a percutaneous stent placement during this hospitalization. He was discharged from hospital the following day. M.M. presents back to your primary care office. He is unsure of the directions that he was discharged with.
