Introduction/Overview

Goals for project development and implementation within the two-week period:

Identify the number of older adult members actively engaging in the blood pressure screening who report to have hypertension

Identify the project deliverables

Identify the resources and tools needed to implement capstone project

Progress Goal #1: There are 18 older adults’ member who regularly participate in the blood screening activity every week and 15 of them reported to have HTN. This validates the relevance and need of the capstone project pertaining to controlling HTN.

Progress Goal #2: After discussion and feedback from the stakeholders, it is determined that the capstone project will be executed with patient teaching and introducing an information corner with brochures/pamphlets regarding HTN management.

Progress Goal #3: With Ms. Ospino’s assistance, the TV room will be used to conduct the patient teaching as there is functioning TV available to present PowerPoint slide show. A functioning speaker and microphone system is also available for clearer speech during teaching. Also, it is decided that the information corner will be placed on the center of the activities room where it is visible and accessible to everyone.

Challenges

One challenge faced is that the program manager will be on vacation starting October until November. As a result, there will be changes in terms of communicating the process and progress of the capstone project. To address this, we discussed that the program manager will be constantly updated through text, call or email. Any urgent issues, needs and concerns will be discussed with the case manager or transport coordinator. This is important as remaining open and communicating progress fosters the spirit of ownership to maintain enthusiasm of the project (Harris et al., 2020, p. 9). Also, the activity calendar for November is being done this week so the schedule for the implementation of capstone project is needed to be identified.

Utilization of Coaching/Mentoring

Interaction with older adults during blood pressure taking is a constant coaching/mentoring moment as this is utilized to reinforce the importance of medication adherence, routine PCP visit and lifestyle modifications in managing HTN. This exemplifies a collaborative, meaningful relationship that enable patients to take the lead in achieving their health goals (Barr & Tsai, 2021).

Capstone Project Plan Progress

Within the two-week period, it is considered that there is not much progress made. Identification of strategies as challenges occur is progressing. Engagement of the team to accomplish goals is maintained and patient education is reinforced for every encounter while trusting patient-nurse relationship develop.

Leadership Lessons Learned

Leadership lessons reinforced within the two-week period is participatory leadership style which builds and fosters a culture of engagement among team members. When the team is actively involved throughout the process of change, they become more productive, motivated and committed to make the project a success. Shared understanding of the desired outcome is promoted as well as team cohesiveness. It nurtures staff members’ ability to learn and develop skills. (Russel, Thomas & Harris, p. 147-148).

Outcomes Achieved

Review of patient self-reported medication list is performed in every interaction and advised patient to see their PCP when discrepancies are observed and to clarify changes or any concerns regarding medication.

Rapport is built and mutual understanding was encouraged among the seniors who are regularly participating in the blood pressure screening activity.

Clinical Hours

Over the past 2 weeks, a total of 10 clinical hours and 4 hours allotted to research were achieved. Activities carried out include ongoing needs assessment, blood pressure monitoring activity and patient teaching regarding importance of adherence to medication regimen, routine PCP visits and lifestyle modifications including diet, exercise and stress reduction activities
