Introduction/Overview

Goals for project development and implementation within the two-week period:

Reviewing and organizing patient education materials and tools received from NYC Health

Ongoing communication with team members to share and reinforce information, provide feedback and update on the progress of the project

Identify approaches for postimplementation monitoring and evaluation

Progress Goal #1: Patient education materials and tools were received from NYC Health They were reviewed to ensure they are applicable to the target population and goals of the project.

Progress Goal #2: Maintaining active engagement and motivation through continually communicating throughout the project development is key to success of the project (Harris et al., 2020, p. 167-168).

Progress Goal #3: Periodic post implementation reviews and monitoring of progress are essential to evaluate quality and sustainability of the project and to ensure that the project is progressing on time and on budget. Performing surveys, interviews and focus groups can evaluate level of knowledge of senior members and to measure level of satisfaction in the services received in terms of hypertension management (p.170-171).

Challenges

One of challenges experienced within the two-week period is the absence of the site manager which impacts the communication process. Even with email and text messages, sharing of information regarding the project including issues and progress and seeking of feedback are challenging and not fulfilled in a timely manner. However, in any improvement projects, setbacks as such are inherent. The most important is that continually communicating among team members is performed to maintain commitment and reinforce shared understanding to achieve goals (p. 167-168).

Utilization of Coaching/Mentoring

Coaching/Mentoring is continuous throughout the project development and planning. It is enlightening to be reminded by Dr. Newton that the purpose of the capstone project is not only development, implementation, evaluation, and presentation of a project that can influence health of the community. It also involves any activities that will foster leadership skills, professional development, group synergy and dynamics skills and personnel management skills which are essential in the nurses’ roles as leaders and agents of change.

Capstone Project Plan Progress

The capstone project is moving according to set timeline and allotted budget. Ensuring that the project’s funds are not misallocated and the goals and milestones of the project are being met are important in its success (p. 172).

Leadership Lessons Learned

As the role of nurses as leaders is reimagined, strong leadership entails active involvement and being a full partner in transforming health care. It also means “taking responsibility for identifying problems and areas of waste, devising and implementing a plan for improvement, tracking improvement over time, and making necessary adjustments to realize established goals” (IOM, 2011).

Outcomes Achieved

Project milestones are met within time frame and allotted budget.

Project activities is continuously monitored and reported.

Project remained malleable to adapt to any changes or issues in the implementation.

Clinical Hours

Over the past 2 weeks, a total of 14 clinical hours with 4 hours allotted to research, 4 hours assigned to IPE meeting, 2 hours allotted to development and completion of Linked in Profile and 4 hours for carrying out activities such as ongoing communication with the team, blood pressure monitoring and individual patient teaching related to hypertension management
