For persons with hypertension, health education and awareness are significant for achieving better health outcomes and improving the health status of the patient. For a group of patients with hypertension, case study illustrations and group education using robust patient education material will easily achieve the desired goals and objectives. The strategies will enable patients to gather extensive knowledge on how to manage their conditions and get answers to various questions they have concerning their condition (Sharma, 2017). Patient education will also share evidence-based practices to patients and how they should implement the practices to achieve the best health outcomes in the self-management of hypertension. Overall, patient education has specific goals and objectives.

In a patient education session for patients with hypertension, the main goals are to explain the significance of lifestyle changes in boosting outcomes and regulation of blood pressure within the desired range. During the session, therefore, the educator will explain the benefits of adhering to a healthy diet with low sodium, weight management, limiting alcohol, regular physical exercise, and stopping smoking will help in lowering blood pressure to the target reading (Mahmood et al., 2019). The other goal will be to compare and contrast signs and symptoms of improved response to therapy and deterioration of the patient’s conditions. the achieve the goal, the educator will focus on objectives such as using case studies covering patient behaviors and lifestyle to demonstrate how signs and symptoms of hypertension can improve or worsen with certain lifestyle changes, therapy compliance and other external factors. By the end of the patient education session, patients will be able to self-manage their blood pressure levels using both lifestyle changes and indicated medications.
