Relapse Prevention Plan
This assignment will assist you, a future counselor, in creating a relapse prevention plan. Relapse prevention plans can be useful tools for clients to refer to throughout counseling and once counseling episodes have been completed.
As a counselor in training, you have been tasked with creating a relapse prevention plan. Using the information found in the “Jed Case Study” document (attached), create that relapse prevention plan for Jed (using template attached). Use third person (i.e., Jed will, or the client will) and assume that the two of you have formulated the plan together.
